Their salary pays for that, plus bonuses and other compensation. Insurance rates are extremely high for no reason. Some medications are even cheaper without insurance. There is no reason the cost should be so high.

Because they bill whatever they want and insurance providers pay it. Hospitals and health clinics are part of the problem. You’d have to regulate how much clinics are billing to insurance a lot more.
