Idk if this is a stretch cuz it’s rlly not romance and it’s only available in audiobook form but Eric Ugland’s Bad Guys LitRPG Series is def off the beaten path & I’m enjoying it! (I think ur actually supposed to start with the Good Guys series but I cannot vouch for that one yet)

Seconding the Radiant Emperor duology (setting is medieval China and it’s explicitly very queer) and Realm of the Elderlings (medieval Europe inspired world and vaguely queer)
