they can be both, delusions (thinking the color of someone’s skin determines their personhood) are a sign of many mental illnesses, and encompass many different disorders. i mean, it’s not a good thing to be mentally ill in the first place (i am also mentally ill) but i agree with the essence of what you’re saying
