Maybe try doing things step by step, like putting on your seat belt and turning on the car etc. until you feel some peace with driving again

I remember when I almost got into a crash, I was petrified to drive again, mind you I was still learning. I started to do really short drives, under the limit, no highway, and from there, little by little, I drove more.
