You still get drops, it's lossy but at that scale it doesn't matter imo.

Yeah but I've always wanted to do a fully overworld rail system... and it helps with moving items around via minecart. I have a nether roof hw but wanna use rails primarily.

Rails still work in the Nether, but I suppose you can’t beat the scenery of an overworld rail system.
