I believe it got made up by people outside of mojang, then rumors did what rumors do, and got more people to “see” him or blame him for chunk errors. There were also a ton of mods that added him in some form, but Im not sure how early in the timeline those were created. afaik, the only thing mojang ever did to play into it was “remove herobrine” every update.
