Go to your university accommodations office/center for student success/whatever they call it

your university accommodation office will get you set up with that. my university did it so i had a meeting with an advisor and she basically just asked me what my biggest struggles were & then went through a bunch of options and was like “what would you like from this list”

Forgot to add to my comment so adding to yours: remember it will be YOUR job to inform professors of your accommodations and which ones you will be taking advantage of, they’ll send an email saying someone in their class has accommodations, but it’s your job to share them with the professor and have that discussion with them
